About The Role
We are seeking a compassionate and skilled RN Case Manager to provide patient-centered end-of-life care in homes, assisted living facilities, and skilled nursing settings. This role focuses on symptom management, emotional support, and ensuring comfort for patients and their families. As a key member of the interdisciplinary care team, you will coordinate services that uphold the highest quality of hospice care.
Key Responsibilities
- Conduct comprehensive patient and family assessments to develop individualized care plans.
- Monitor patient conditions, manage symptoms, and provide direct nursing care in home and facility settings.
- Administer medications and treatments in alignment with physician-directed plans of care.
- Collaborate with an interdisciplinary team, including physicians, social workers, chaplains, and aides, to provide holistic care.
- Educate families and caregivers on symptom management, medication administration, and end-of-life care practices.
- Maintain thorough and timely documentation of patient status and interventions.
- Communicate effectively with physicians and care teams regarding changes in patient conditions.
- Participate in on-call rotations to ensure 24/7 access to hospice support.
- Registered Nurse (RN) with an active license in the state.
- Two or more years clinical experience with one year in the acute care setting preferred.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills for working with patients, families, and care teams.
- Ability to travel locally within a designated radius.
