Job Summary:
The LVN (Licensed Vocational Nurse) Intake Coordinator is responsible for coordinating the intake and admission process for new home health patients. This position ensures timely and accurate processing of referrals, verification of patient eligibility and benefits, and initiation of care in collaboration with clinical and administrative teams.
Key Responsibilities:
- Receive and process incoming referrals from hospitals, physicians, case managers, and other referral sources.
- Verify patient eligibility, insurance coverage, and authorization requirements.
- Collaborate with clinical and administrative staff to schedule initial assessments and start-of-care visits.
- Communicate effectively with referral sources, patients, families, and staff regarding admission status and care planning.
- Enter and maintain accurate patient information in the electronic health record (EHR) system.
- Ensure all required documentation is obtained and meets regulatory, payer, and agency requirements before admission.
- Coordinate with nursing and therapy teams to assign appropriate clinicians based on patient needs and availability.
- Provide education to patients and families regarding services offered, expectations of care, and agency policies.
- Track and follow up on pending referrals to ensure timely admission and continuity of care.
- Participate in team meetings and contribute to process improvement initiatives.
- Maintain compliance with HIPAA regulations and company confidentiality policies.
Job Qualifications:
- Current and active LVN license in [State].
- Minimum of 1–2 years of nursing experience, preferably in home health or a similar healthcare setting.
- Knowledge of home health regulations (Medicare, Medicaid, and private insurance) is preferred.
- Strong communication and organizational skills.
- Proficient in EHR systems and Microsoft Office (Word, Excel, Outlook).
- Ability to multitask in a fast-paced environment.
- Detail-oriented with excellent time management skills.
Working Conditions:
- Office-based with occasional remote work depending on agency policies.
- May require sitting for long periods, as well as occasional lifting of files or office supplies.
- Must be able to work flexible hours as needed to meet referral and patient care demands.
