JOB PURPOSE:

The IDT Facilitator and Audit Lead, will coordinate the process of care planning for our participants. Will manage the schedule for care planning and assign cases to the interdisciplinary team to care plan after they have verified all required elements have been completed.

The IDT Facilitator and Audit Lead is responsible for ensuring that individualized care planning is completed for each of our participants in a timely manner following PACE regulations. Will also be instrumental in completion of audits including but not limited to timeliness of IDT documentation, Participant involvement in plan of care, and my needs card expenditures.

JOB RESPONSIBILITIES:

  • Maintains a schedule for care plan completion for assigned sites.
  • Monitors timeliness of documentation and escalates cases where late assessments and/or documentation are holding up care plan completion.
  • Ensures timely facilitation of care planning by ensuring each member of the IDT provides required assessments, planning, care coordination, implementation, advocacy, and evaluation/oversight necessary for all aspects of health care delivery to CenterLight PACE Participants.
  • Ensures attestation form is completed and signed by all disciplines and participants on case conference.
  • Participates in quality meetings with the site-associated leadership team to ensure quality standards are met.
  • Responsible for assigning participants to various IDT members in Care Compass for assigned PACE sites.
  • Works with the other departments to resolve any possible issues. Takes initiative, is resourceful, proactive, creative, innovative, and independent.
  • Suggests and implements patient-centered service improvements.
  • Lead completion of audits including but not limited to timeliness of documentation, Participant involvement in the plan of care, and review of my needs card expenditures
  • Conducts weekly and ad-hoc reports to monitor care plan and IDT visit productivity.
  • Participate in special projects and assignments as required.
  • Only act within the scope of the individual’s authority to practice.
  • Meet a standardized set of competencies for the specific position description established by the PACE organization before working independently.

Schedule: 8:30AM – 5:30PM

Weekly Hours: 40

QUALIFICATIONS:

Education:

  • A Baccalaureate degree is highly preferred.
  • Project Management Certifications, preferred.

Experience:

  • Customer Service experience required.
  • Demonstrate proficient typewriting skills. Typewriting test must be taken with a minimum of 45 words per minute preferably, and over 90% accuracy.
  • One (1) year of general experience in health care preferred.
  • Minimum of one (1) year of experience working with a frail or elderly population or, if the individual has less than one (1) year of experience but meets all other requirements, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
  • Supervisory experience preferred.

  • Is self-directed with the ability to work with or without supervision.
  • Demonstrated ability to establish a good rapport with participants and peers is essential.
  • Tactful and courteous.
  • Pleasant telephone manner.
  • Must be able to work weekends if required.
  • Communicates with coworkers and superiors in a professional yet caring, considerate, and culturally sensitive manner.
  • Excellent written and verbal communication.
  • Proficient in use of a computer for email, word processing, research, and maintenance of Participant records.
  • Be legally authorized (for example, currently licensed, registered, or certified if applicable) to practice in the State in which the healthcare professional will perform the function.
  • Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.

Physical Requirements

Individuals must be able to sustain certain physical requirements essential to the job. This includes, but is not limited to:

  • Standing – Duration of up to 6 hours a day.
  • Sitting/Stationary Positions – Sedentary position in duration of up to 6-8 hours a day for consecutive hours/periods.
  • Lifting/Push/Pull – Up to 50 pounds of equipment, baggage, supplies, and other items used in the scope of the job using OSHA guidelines, etc.
  • Bending/Squatting – Have to be able to safely bend or squat to perform the essential functions under the scope of the job.
  • Stairs/Steps/Walking/Climbing – Must be able to safely maneuver stairs, climb up/down, and walk to access work areas.
  • Agility/Fine Motor Skills - Must demonstrate agility and fine motor skills to operate and activate equipment, devices, instruments, and tools to complete essential job functions (ie. typing, use of supplies, equipment, etc.)
  • Sight/Visual Requirements – Must be able to visually read documentation, papers, orders, signs, etc., and type/write documentation, etc. with accuracy.
  • Audio Hearing and Motor Skills (Language) Requirements – Must be able to listen attentively and document information from patients, community members, co-workers, clients, providers, etc., and intake information through audio processing with accuracy. In addition, they must be able to speak comfortably and clearly with language motor skills for customers to understand the individual.
  • Cognitive Ability – Must be able to demonstrate good decision-making, reasonableness, cognitive ability, rational processing, and analysis to satisfy essential functions of the job.

Pay: $55,000.00 - $60,000.00 per year
