River Edge offers behavioral health outpatient services to adults in Macon-Bibb County experiencing mental health and substance use challenges and is looking to fill the position of Admissions Care Coordinator with/at our Bibb Outpatient Services program.
In this role you will:
Provide care coordinator of individuals receiving services through River Edge Behavioral Health to ensure that individuals' behavioral health, physical health, and other identified needs are met and that services are coordinate with primary care and other systems with which the individuals is involved. Primarily interfaces with the Admissions Clinicians to ensure appropriate referrals and coordination is maintained with internal and external service providers.

Responsibilities:
  • Provides care coordination services for all newly admitted clients working in collaboration with the Admissions Clinician.
  • Work collaboratively with clients to obtain demographic information.
  • Enters all client information into the Electronic Medical Record.
  • Participates in assessment activities and the development of person-centered treatment plan.
  • Engages in follow-up activities with internal River Edge programs, community partners, and primary care organizations to ensure quality care.
  • Responsible for monitoring service utilization and outcomes data based on identified target areas.
  • Complete follow up activities with clients to ensure all referrals are completed and client engages in services to all referred programs.
  • Interface with staff of River Edge programs and Admissions Clinicians to assure good communication, problem resolution, quality service provision, and ease of transition into services.
  • Provide consultation to River Edge team members and community partners regarding alternatives, creative approaches to care, etc.
  • Responds to all client questions and concerns.
  • Identifies and reports any clinical, utilization, or outcomes issues to appropriate staff.
  • Participate in quality improvement activities and professional development activities.
  • Participate in clinical supervision, staff meetings, and other meetings as required.
  • This position will be knowledgeable about and actively support 1) culturally competent, recovery-based practices, 2) person centered planning as a shared decision-making process with the individual who defines his or her own life goals and is assisted in developing a unique path towards those goals, and 3) a trauma informed culture to aid consumers in the recovery process.

You will need:
  • A minimum of a bachelors degree in social work, psychology, criminal justice, or other healthcare related field
  • A strong understanding of the needs of persons with mental illness, intellectual/developmental disabilities, and substance use disorders.
  • Understanding of co-morbid primary care conditions and understanding of how to implement as strong recovery philosophy.
  • Interpersonal and human relation skills with an ability to collaborate with teams.
  • Proficient in Microsoft Office
  • Positive energy and excellent communication skills
  • Acceptable background check/drug screen
  • Acceptable references

Preferred Qualifications:
  • Must have the flexibility to assist other departments as needed
  • Demonstrate knowledge in mental health and substance abuse services and community resources.
  • Customer service experience or experience in a behavioral health setting.

Location/Schedule:
175 Emery Hwy, Macon, GA 31217.
Monday - Friday: 8:30am - 5:00pm.
