About Job

DUTIES/RESPONSIBILITIES:

Manage a caseload of clients who have permanent disabling conditions, such as chronic and acute mental illness or serious and disabling medical conditions that preclude them from employment. Meet weekly with each client, linking clients to medical and mental health services in the community, maximizing the receipt of appropriate entitlements, and facilitating placements into supportive housing. Complete NY/NY housing applications and assist clients with all aspects of the supported housing process. Conduct therapeutic and psycho-educational groups covering broad array of topics such as stress management, impulse control, living with chronic mental illness and decision- making processes. Related duties as assigned.

HOURS:

Full-time, 37.5 hours per week

  • Sunday-Thursday 8am-4:30pm

QUALIFICATIONS:

Related Associates degree required. Related Bachelor's or Master's degree preferred. Experience working with the mentally ill/medically frail populations preferred. CPR training certification or willingness to take training class in CPR. Basic computer literacy required.
