Alamo Health Management Santa Clarita provides transitional housing and resident-centered care services for clients suffering from mental health, addiction, and/or dual diagnosis. The ideal candidate wants to help break the cycle of clients moving from crisis centers to housing instability and back to crisis centers.
This role is responsible for managing a large caseload and resident's needs with regards to substance use and harm reduction. Their duties include meeting with clients 1:1 when individual SUD support is indicated, as identified by the team. This role involves assessments, group facilitation, providing 1:1 support and communicating/collaborating with multiple members of the resident's care team.
Essential Functions
- Assist in the daily needs of residents including the participation in interdisciplinary team meetings.
- Provide observation, ongoing assessment and intervention consistent with physical and psychological status.
- Provide clients with individual and group SUD support in order to achieve recovery goals.
- Participate in the care plan meetings, initial assessment, level of care considerations, and ongoing SUD support as needed.
- Will keep an individual caseload
- Support clients and leadership with utilizing Harm Reduction principles when relapses occur, ensuring continued de-stigmatization of substance use, and providing necessary treatment to reduce use in the program community.
- Perform other duties as assigned or needed.
Education and Experience
- CCAPP Certification (Required)
Certifications, Licenses, and other Special Requirements
- Certifications as required by state.
- Must meet all health requirements, including TB.
- Must pass criminal background check.
- Must have compassion for and desire to work in a mental health environment.
Essential Skills
- Excellent verbal and written communication skills. Able to communicate verbally in person and over the phone in fluent English. Able to read, analyze, interpret and complete documentation in fluent English.

Job Type: Full-time
Pay: $66,000.00 - $81,000.00 per year
