Blize Healthcare is looking for a Hospice Community Liaison to join our team.

Responsibilities:

Developing and maintaining relationships with healthcare providers, community organizations, and other referral sources.
Promoting hospice services through sales calls, trade shows, and other activities.
Informing the community about hospice/home health care, services, and benefits.
Tracking and reporting referral and admission data to identify trends and growth opportunities.
Working with the clinical team to ensure a smooth transition to hospice/home health care.

Required Skills & Qualifications:

Bachelor's degree preferred.
Minimum one year of sales/marketing experience in healthcare.
Ability to market collaboratively and productively with customers, referral sources, and the community.
Excellent written and verbal skills, computer literate.
Strong organizational, people, and problem-solving skills.
