SUMMARY RESPONSIBILITIES:

Responsible for case management of clients participating in 3Strands Global Foundation’s (3SGF) Employ + Empower program (E+E), serving survivors of trafficking and at-risk individuals primarily in the Greater Sacramento Region. The Case Manager will provide services and resources to individuals to ensure their basic needs are met and barriers to employment and education are addressed while working with Workforce Development specialists, Education Specialist, and external employment partners to facilitate placement into employment or education opportunities. The Case Manager will collaborate with the Program Manager and Director of Employ + Empower to ensure that clients who are placed in jobs have access to services and resources and are receiving ethical and trauma-informed support.

SPECIFIC RESPONSIBILITIES:

Case Management

  • Provide direct case management to survivors of trafficking and at-risk youth to ensure basic needs and behavioral health needs are met in a timely and culturally appropriate manner for each Employ + Empower program participant, including:
  • Collaborating with the Program Manager to facilitate the client intake process and complete a needs assessment for each client.
  • Meeting with clients on a weekly/biweekly basis to coordinate services as needed to assist with basic needs and other needs that relate to employment and education to assist clients in sustaining employment and education, including transportation, mental health services, housing, professional clothing, professional certifications, identification documentation, childcare, medical services, social service programs, etc.
  • Research and coordinate necessary referrals to partnering agencies and ensure clients access can access services and resources.
  • Provide crisis intervention support to clients as needed.
  • Collaborate with partner agencies to facilitate the exit plan and next steps for clients after participation in the Employ + Empower program.
  • Weekly check-ins and reporting to Reintegration Program Manager on client progress.
  • Work alongside the Workforce Development Specialist and Education Specialist to ensure appropriate and supportive job placement.
  • Assist with work readiness and financial literacy training for clients as needed.
  • Communicate with referring agencies to collaborate on services and provide status updates on clients’ progress in the program.
  • Collaborate with the Reintegration Program Manager to build partnerships with local agencies and service providers.

Administrative Responsibilities

  • Follow the established program process to maintain accurate and timely case file documentation to reflect the needs of the individual client, including the maintenance of clear and concise case notes, documenting services provided, and completing all required documentation to ensure client progress is documented and accurately tracked.
  • Complete required monitoring and evaluation documents and qualitative interviews among clients to monitor their progress in desired program outcomes;
  • Attend meetings as needed to advance reintegration efforts, including local coalition meetings, events, and training;
  • Participate in all program meetings, staff meetings, strategic offsite, and development activities and events, and fully engage as a member of the 3Strands Global Foundation team.
  • Collaborate with Reintegration Program Manager to review and provide input about E+E services and processes.
  • Track time and program expenses and mileage with MileIQ, to ensure accurate budgeting and grant reporting.
  • Assist with grant reporting on program objectives and accomplishments.
  • Complete reports to submit to The California Department of Rehabilitation and Fresh Successreflecting services provided.
  • Complete invoicing as needed.
  • Coordinate with clients to speak on behalf of the reintegration program at 3Strands GlobalFoundation fundraising events.
  • Support all marketing, community, development, and outreach efforts by supporting the gathering of information and client testimonials and providing input for the creation of presentations and collateral.
  • Ensure the confidentiality of clients is upheld.
  • Comply with all policies, procedures, and protocols of the agency; and
  • Other related duties as assigned.Development
  • Add grants and or development opportunities to Monday board for 3SGF growth.
  • Share social media and posts with network.
  • Attend networking events to grow 3SGF
  • Attend events, trade shows or speaking events as needed for 3SGF

QUALIFICATIONS:

  • Case manager I: bachelor’s degree in social work or related field and/or 0-3 years' experience in social services
  • Case manager II (Social Worker I - if the person holds an MSW): master’s degree in social work, less than 3 years’ experience post-master's program or 4-6 years case management experience
  • Case manager III (Social Worker II - if person holds an MSW): bachelor's degree or master's degree +4 years’ experience post-graduation or 7+ years’ experience case management
  • Survivors of human trafficking and individuals with lived experiences are encouraged to apply.
  • Previous multicultural experience and demonstrated ability to communicate well with people of varied backgrounds are strongly preferred.
  • Prior experience using client data and tracking systems is strongly preferred.
  • Must have a valid driver’s license, a reliable vehicle with current insurance, and the ability to travel regularly throughout the service area.
  • Proven ability to manage and nurture relationships.
  • Solid understanding of the nonprofit sector.
  • Proven ability to work well in a team environment;
  • Solid understanding of human trafficking, Commercial Sexual Exploitation of Children, and trauma-informed care.

DESIRED SKILLS:

● Bilingual.

● High proficiency with Microsoft Office applications (Word, Excel, Outlook) and Google Suite,Monday.com, and other software and technology.

● Attention to detail and ability to organize and track large amounts of information efficiently andaccurately.

● Sound judgment, ability to take initiative and provide constructive feedback, and analytical andproblem-solving skills.

WORKING ENVIRONMENT:

  • In-office and field time with clients in and around the service delivery area.
  • Travel in and around the service delivery to deliver and coordinate services for clients, accompanying clients to meetings and appointments.
  • May require occasional weekend and/or evening work for 3Strands Global Foundation events.
