Qualifications:

Must possess a Bachelor's degree from an accredited college or university with a major in Psychology, Social Work, or related mental health field.
Must possess a valid Michigan motor vehicle operator's or chauffeur's license.
Driving and Auto Insurance

Duties and Responsibilities:

Functions as the pathway coordinator by assisting with outreach, coordination and screening for children/families referred to a pathway of care.

Assists with outreach to families to explain services provided.

Provides appropriate screening for pathways indicated by intake.

Collaborates and serves as an integral part of the clinical pathway team[1]based care model.

Initiates and monitors National Outcomes Measurement Survey (NOMS) to those assigned to care pathway.

Outreaches to families to support continued screening or outcomes.

Demonstrates knowledge of principles of trauma informed care, suicide risk and substance use.
