Job Summary

We are seeking a motivated Home Health Community Liaison (Marketer) to join our team. The ideal candidate will be responsible for building and maintaining relationships with hospitals, skilled nursing facilities, physician offices, case managers, and social workers to drive referrals and admissions. This role requires strong communication skills, local market knowledge, and a proven ability to grow referral sources.

Responsibilities

• Develop and maintain referral relationships with hospitals, SNFs, physician offices, and other community partners
• Represent the agency professionally and compliantly in the community
• Educate providers on our services, coverage areas, and insurance acceptance
• Track daily visits, calls, and referrals; provide weekly activity and results reports
• Collaborate with intake and clinical staff to ensure smooth admissions
• Attend community events and in-services to promote the agency

Requirements

• 1+ year of home health, hospice, or healthcare marketing experience preferred
• Existing relationships in Los Angeles County a strong plus
• Valid CA driver’s license and reliable transportation
• Excellent communication and interpersonal skills

Compensation

• Base salary: $55,000 – $70,000 annually (DOE)
• Monthly commission based on performance (compliance-friendly, not per referral)
• Total earning potential: $70,000 – $100,000+ annually
