Key Responsibilities
· Oversee and manage the day-to-day operations of the social services department, ensuring high standards of care and service delivery.
· Develop, implement, and evaluate social service programs to meet the psychosocial needs of residents and their families.
· Provide counseling, support, and advocacy for residents facing adjustment, emotional, or discharge planning challenges.
· Lead and participate in interdisciplinary care planning meetings, ensuring that social service goals are integrated into comprehensive resident care plans.
· Facilitate resident admissions, transfers, and discharges, coordinating with residents, families, and external agencies.
· Ensure the department's compliance with all federal, state, and local regulations pertaining to social services in long-term care.
· Supervise, mentor, and evaluate social work staff, fostering a collaborative and high-performing team.
· Develop and maintain strong relationships with community resources to facilitate appropriate referrals and support services for residents.
· Address and resolve resident and family concerns, promoting open communication and satisfaction.
Qualifications & Experience
· Master's Degree in Social Work (MSW) from an accredited program required.
· Current New York State license as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).
A minimum of 3-5 years of direct social work experience in a skilled nursing facility or long-term care setting, with at least 2 years in a supervisory or leadership role.
