Job Title: Social Worker, MSW (Direct Hire)

Location: El Cajon, CA 92020
Schedule: Monday – Friday, 8:00 AM – 5:00 PM
Work Setting: On-Site with occasional patient home visits
Compensation: $37.05 – $42.03 per hour

Job Summary:

We are seeking a compassionate and dedicated Social Worker (MSW) to join our team at the. This role focuses on providing comprehensive social services to a senior population, ensuring participants receive the support needed for physical, emotional, and social well-being.

Key Responsibilities:

• Conduct psychosocial assessments and develop individualized care plans for participants.
• Provide counseling, crisis intervention, and advocacy for participants and their families.
• Coordinate with the interdisciplinary care team to ensure holistic care.
• Facilitate access to community resources, benefits, and support services.
• Document assessments, care plans, interventions, and progress notes accurately.
• Participate in care plan meetings and interdisciplinary team discussions.
• Perform home visits as needed to assess participant needs and provide services.

Required Qualifications:

• Education: Master’s Degree from an accredited School of Social Work (MSW).
• Experience:
• Experience with elderly or frail populations preferred but not required.
• Candidates should be comfortable working directly with seniors and patients.
• Licenses/Certifications:
• Valid California Driver’s License.
• Reliable, insured transportation in compliance with center guidelines.
• Language Skills:
• Bilingual in Arabic highly desired.
• Bilingual English/Spanish or English/Arabic strongly preferred.
