Preferred Care at Wall is a skilled nursing and rehabilitation facility dedicated to delivering high-quality, compassionate care to our residents. We are currently seeking a Licensed Social Worker for a temporary assignment to support our social services department during a staff leave period.

Beginning October 2025

Position Overview

The Temporary Social Worker will work closely with our interdisciplinary team to provide psychosocial support, discharge planning, and advocacy for residents and their families. This role is crucial to maintaining continuity of care and ensuring regulatory compliance.

Key Responsibilities

• Conduct initial and ongoing psychosocial assessments
• Participate in care plan meetings and interdisciplinary care conferences
• Coordinate discharge planning, referrals, and family communication
• Address resident concerns and provide emotional support
• Maintain accurate documentation in compliance with state and federal guidelines
• Assist with grievance processes and resident rights advocacy

Qualifications

• Required: Licensed Social Worker (LSW or LCSW) in the State of New Jersey
• Bachelor’s or Master’s degree in Social Work (BSW/MSW)
• Experience in long-term care, skilled nursing, or geriatric settings preferred
• Strong organizational and communication skills
• Ability to work independently and as part of a collaborative care team

Schedule & Compensation

• Flexible schedule: Full-Time
• Day shifts (Monday–Friday)
• Competitive hourly rate based on experience
• Potential for assignment extension or permanent placement

Job Type: Temporary

Pay: $47,873.86 - $57,654.54 per year
