We’re looking for a dedicated Patient Care Coordinator In this role, you'll be responsible for ensuring clients receive timely, personalized care by coordinating and scheduling staff based on individual care plans and preferences. You’ll manage caregiver schedules, handle inbound calls with empathy and professionalism, and maintain accurate records in scheduling systems. Your ability to build strong relationships with clients and families—and collaborate with internal teams—will help create a seamless care experience.
Responsibilities:
- Coordinate and schedule care staff based on clients’ individual needs, preferences, and care plans
- Provide excellent customer service by handling inbound calls with empathy, professionalism, and efficiency
- Build and maintain strong, supportive relationships with clients and their families
- Collaborate with internal teams to address staffing needs and changes
- Track and manage caregiver schedules and timecards; ensure timely and accurate reporting
- Maintain detailed records and update scheduling systems regularly
- Foster a positive, team-oriented environment aligned with the company’s mission and values
- Ensure all scheduling and staffing practices comply with company policies and healthcare regulations
Qualifications:
- Home healthcare experience required
- Bilingual in Spanish is required
- Excellent organizational and time management skills
- Strong verbal and written communication abilities
- Detail-oriented with the ability to multitask in a fast-paced environment
- Proficient in Microsoft Office Suite (Word, Excel, Outlook, PowerPoint)
