- Coordinate transitions of care for patients to reduce emergency department visits and readmissions.
- Perform medication reconciliation and ensure accurate medication management.
- Manage all inpatient discharges, excluding labor and delivery and oncology follow-ups.
- Focus on emergency department visits with an emphasis on patients with chronic conditions.
- Case management experience.
- Proficiency in EPIC software.
- Active RN license in the state of Washington.
- Experience in care coordination.
- 1+ years of experience in case management within an acute hospital, long-term acute care, rehab, or skilled nursing setting.
- Basic Life Support (BLS) certification.
- Experience with utilization review and discharge planning.
- Knowledge of population health and interqual.
