About Job

As a patient care coordinator you are able to be the patient's clinical advisor, advocate, and help to improve their quality of life. You will be working with a team of LPN's/MAs to coordinate their referrals, assist with paperwork, patient education, and review preventative medicine.

Position Summary:

Improving Patient Care Together: We help high-risk patients get the most out of life. Our care coordinators are in constant contact with the patient via phone resulting in boosting patients' self-care abilities and keeping them well. We team up with their families, caregivers, specialists, and community resources to optimize the patient's experience and well being.

Job Duties:

  • Patient advocate.
  • Work as a team player to ensure each patient receives high quality care.
  • Provide community resources and referrals for continuing care.
  • Helping patients complete paperwork
  • Educating patients about their care
  • Monitoring a patient's progress toward their healthcare goals.
  • Solving a wide array of problems when they arise.
  • Collaboration to meet Hedis data collection goals
  • Performs other duties as assigned by the supervisor.

Job Type: Full-time

Pay: $18.00 - $25.00 per hour
