Job Overview:

AMI, Inc. is seeking a reliable and responsible Supportive Housing Case Manager to join our team. The SH case manager will assist participants in obtaining and/or maintaining appropriate housing using the Psychiatric Rehabilitation model. This is a higher-level position that requires extreme organizational skills, time-management, and the ability to be independently productive.

Job Duties

  • Assist participants in identifying their personally chosen goals through an assessment of strengths and needs
  • Implement structured activities and interventions such as role play, modeling, and direct skills teaching that will assist participants to achieve specific living, learning, working, and socializing goals
  • Help participants identify barriers to goal completion and organize a plan of action to overcome them
  • Complete all necessary documentation in a timely manner

Qualifications:

  • Personal transportation and willing to use personal vehicle for work as required
  • Must possess a valid PA driver’s license and be fully insurable in the state of PA
  • Must be a prior/current mental health consumer with the ability to role model recovery
  • Successful candidates must undergo a drug screening as part of the hiring process and maintain a drug-free status throughout employment
  • Candidates are subject to state and federal background checks (certain charges related to violence or offenses concerning the welfare of children may disqualify applicants for employment)
  • Educational Requirement – one of the following:

o Bachelors Degree or equivalent housing experience preferred

o Associates Degree + one-year experience in mental health direct services

o Certified Peer Specialist + one year additional paid/volunteer work experience

o High School Diploma/GED + 2 years experience in human services which must include 1-year mental health direct care experience

Job Type: Full-time

Pay: $18.00 - $21.00 per hour

Expected hours: 37.5 per week
