Eliot's Family-based Intensive Treatment (FIT) services will offer short term intensive treatment for higher acuity children, youth and families for whom more traditional outpatient services may not be effective or accessible. A FIT Team is composed of highly trained Clinicians, Care Managers and Family Partners, that will work closely with a family at least three times each week, including individual, caregiver and family sessions, skills coaching and care coordination support. The FIT Team will be trained to deliver innovative and evidence-based clinical care anchored in Values Based Transdiagnostic treatment.
The ideal candidate for the Family-Based Intensive Treatment (FIT) Care Manager is a compassionate and family-focused professional with a background in child, youth and family mental health, crisis intervention, and care coordination within complex service systems. They are highly organized, and skilled in building collaborative relationships with families, providers, and community partners to deliver strengths-based, culturally responsive support.
Responsibilities:
- Serve as key contact for the youth and family to the FIT team.
- Engage in all face to face intake and assessment related activities including collection of information from the family and any/all involved providers and collaterals.
- Develop, review and revise assessments, values-based treatment plans and comprehensive safety and crisis management plans in conjunction with other FIT Team members.
- Initiate care planning activities
- Provide phone outreach to new referrals
- Provide targeted case management and care coordination as clinically indicated through phone calls, in-person meetings, and virtual meetings.
- Support the coordination of care between other behavioral health medical providers and relevant collaterals including but not limited to schools, DCF, DMH, DDS, DPH, DESE/LEA, DYS, Probation Office, Courts, medical providers, etc. to ensure clear communication, shared understanding of family needs and preferences, and treatment
- Provide emergency response and intensive outreach to families in order to assist them in stabilizing children and adolescents within their home during a psychiatric crisis.
Qualifications:
- Bachelor's degree in human services or related field
- Two (2) years' experience working with the target population
- Experience in navigating child/family service systems and experience advocating for family members who are involved with behavioral health systems.
- Must have valid driver's license and reliable transportation to and from work and for providing services in the community
Schedule: Hours of program are 8am-8pm. 40 hours per week schedule to include some evening hours and participation in a rotation of weekend coverage and after hours on-call.
