Our Partial Hospitalization Program (PHP) in San Jose is designed to provide trauma-informed, whole-person care for individuals seeking recovery from substance use disorders and co-occurring conditions.

We are seeking a compassionate and motivated Case Manager / Substance Use Treatment Counselor to join our PHP team. This role is focused on supporting veterans, first responders, and community members who are navigating recovery, trauma, and co-occurring disorders.

Job Summary

The Case Manager/Substance Use Treatment Counselor will provide both group and individual counseling, trauma-informed care, and case management services to PHP clients. This includes guiding clients through structured daytime programming, coordinating care, and helping them access resources to support long-term recovery. In addition, the position may require some administrative tasks such as attendance tracking, supporting urine analysis (UA) testing, maintaining records, assisting with scheduling, supporting program operations, and providing transportation as needed.

Key ResponsibilitiesCounseling & Case Management

  • Provide case management and counseling services for clients with substance use, trauma, and co-occurring mental health disorders.
  • Facilitate PHP group recovery sessions with a trauma-informed, culturally competent approach.
  • Develop and update individualized treatment and case management plans.
  • Assist clients in accessing community resources and referrals, including medical, housing, and peer-support services.
  • Collaborate with clinical teams and outside providers to coordinate individualized care plans.
  • Conduct intakes, assessments, treatment planning, and ongoing progress documentation.

Crisis Intervention & Clinical Support

  • Redirect and de-escalate crisis situations, conduct emergency management, and follow proper emergency procedures.
  • Identify early signs of client emotional distress or relapse risk and intervene appropriately.
  • Participate in multidisciplinary staff meetings and present updates on caseload progress.
  • Support urine testing, breathalyzer checks, and medication monitoring when needed.

Administrative & Program Support (As Needed)

  • Track and document client attendance for PHP programming.
  • Conduct and record urine analyses (UAs) and breathalyzer checks in accordance with policy.
  • Maintain accurate and timely documentation, including treatment plans, progress notes, attendance logs, and incident reports.
  • Assist with scheduling, data entry, and other program-related administrative tasks.
  • Provide client transportation as needed for program-related appointments or services.
  • Assist with operations tasks as needed, including maintaining cleanliness of the facility and ensuring a safe environment.
  • Support overall program operations and compliance with HIPAA, state, and federal regulations.

Qualifications

  • At least 1 year of experience in substance abuse counseling or case management.
  • Knowledge of addiction, co-occurring disorders, trauma, and recovery processes.
  • Strong team collaboration and communication skills.
  • CPR, First Aid certification, and current TB test required as pre-onboarding requirements.
  • Strong writing and documentation skills.
  • Must be registered, certified, or licensed with a California-recognized certifying organization (CADC, CATC, LAADC, or equivalent).
  • Valid California Driver’s License required; ability to safely transport clients when needed.
  • Ability to work effectively in a fast-paced, high-change environment with strong adaptability and flexibility.
  • Experience working with first responders and/or veterans strongly preferred.

Schedule

  • Monday – Friday
  • Daytime PHP hours (approx. 9:00 AM – 5:00 PM).
  • Occasional flexibility may be required based on client needs.
