We are seeking a Service Coordinator to join our team. This pivotal role involves coordinating services for individuals with disabilities and our older adult population, ensuring they receive the support necessary to enhance their independence and quality of life.

The Service Coordinator’s responsibilities are:
  • Development of the person-centered service plan that address a Participant’s identified areas of need, goals, intervention, and progress of goals.
  • Coordinate services for Participants in relation to their identified needs.
  • Monitor authorized services through face-to-face in-home visits, phone calls, person centered planning team meetings, and contact with service providers.

The Service Coordinator duties are performed remotely from home and will require intermittent travel to conduct in-person home visits throughout Delaware County, Pennsylvania.

Must have a bachelor’s degree in social work, psychology or other related fields with practicum experience OR in lieu of a bachelor’s degree, must have at least three (3) years’ case management experience in a social service or healthcare-related setting.
