Social Work Assistant – Hospital Discharge Planner

• Shift: Day Shift (8:00 AM – 4:30 PM), 5x8s
• Contract Length: 13 weeks

Requirements:

• 1+ year of experience (discharge planning, case management, or social work)
• Epic charting experience
• Community hospital background
• Travel experience required
• BLS preferred
• LVN or unlicensed with relevant experience welcome

Job Types: Full-time, Contract

Pay: $36.00 - $37.00 per hour

Expected hours: 40 per week

Work Location: On the road
