HRSA GRANT FUNDED ROLE: End date - August 31, 2026

Work schedule: Monday - Friday, 9am - 5pm with one late night on a Monday, Tuesday or Thursday- 11am - 7pm

Position Overview: We are seeking a Licensed Mental Health Clinician (LMSW, LCSW) to join our Community Support Therapeutic Team. The Mental Health Clinician will provide therapeutic counseling and support to patients experiencing drug or alcohol dependency, educate families in the best ways to help in the recovery process, and work with both patients and families to promote optimum mental health. The services provided to the patients will include mental health and substance use assessments, crisis intervention, individual and group therapeutic counseling, psychoeducation, and referrals. The Clinician will also carry a small caseload. This work is carried out in support of the mission and goals of Ryan Health.

Essential Functions: The essential functions of the Community Outreach Clinician are to provide mental health and substance abuse screenings, including the SBIRT, crisis intervention, and supportive counseling to members in the community who are identified as experiencing mental health and substance use challenges. This Clinician will also provide individual, family and group psychotherapy to a small caseload of Ryan Health patients.

  • Maintain an assigned caseload of patients with diverse mental health and substance use needs.
  • Must demonstrate the ability to provide quality, patient-centered, evidence-based treatment to individuals, groups and families who experience mental health and substance use disorders.
  • Must demonstrate the ability to provide the SBIRT model of screening.
  • Must have experience with relapse prevention, reduction of alcohol and cannabis use, and smoking cessation.
  • Must have experience administering mental health and substance use screenings, treatment and discharge planning, and making quick clinical assessments and referrals to licensed substance use disorder treatment programs.
  • Traveling with the Community Mobile Team on designated days, at least twice a week, is essential.
  • Providing psychoeducation about harm reduction methods and treatment options.
  • Coordinating care with Medically Assisted Treatment providers, as well as others providing treatment to patient.
  • Maintain professional competence by attending professional conferences, trainings, in-service seminars and other trainings as assigned.
  • Participate in staff meetings and case conferences with mental health teams throughout the network.
  • Participate in weekly individual or group supervision
  • Adhere to all agency and departmental policies and procedures.
  • Must demonstrate a professional, courteous, and respectful attitude in dealing with patients, immediate supervisor, and other employees.

  • Immediately report any problems or unusual occurrences to supervisor/s
  • Perform other duties/special assignments as assigned.


Qualifications

Minimum Experience and Skills Required:

  • Two years in a health/behavioral health setting as a Social Worker
  • Two years working with individuals experiencing substance abuse
  • Effective communication, listening, problem solving, and writing skills
  • Excellent Crisis Intervention Skills
  • Ability to travel with the Community Mobile Team at least two days a week.
  • Strong engagement skills
  • Proven track record of organization skills, attention to detail and ability to respond promptly to requests
  • Excellent written and oral communication skills
  • Strong computer literacy with proficiency in standard office software/hardware such as Microsoft Word, Excel, Outlook, PowerPoint.
  • Demonstrate cultural competency

Education, Licenses and/or Certifications Required:

  • Licensed Clinical or Licensed Master's Social Worker (LCSW or LMSW)

Preferred Qualifications:

  • Bilingual
  • CASAC

This position requires moderate physical effort, involving transporting medical equipment, assisting in ambulatory procedures, and lifting patients. This position requires standing 75% of the time. Work Environment

The work is primarily in a mental health clinic and/or general primary care setting which includes the unpredictability in behaviors of individuals and acute and chronic infectious diseases which may be contagious. Adequate preparation and precaution is necessary. Environmental conditions include exposure to airborne infection associated with health care settings; exposure to blood borne pathogens and biologicals; possible exposure to laboratory chemicals and diagnostic radiology materials.
