Job Overview – Mental Health Counselor (LMHC):

Compensation: $40/hour

Location: Brooklyn, NY

Schedule: Monday to Friday, 9:00 AM to 5:00 PM

Provide therapeutic support and care coordination as a Mental Health Counselor (LMHC) with our client in Brooklyn, NY. In this community-based role, you will conduct assessments, develop service plans, and deliver ongoing counseling to clients with mental health and co-occurring conditions. You’ll also facilitate wellness groups, ensure treatment adherence, and connect clients to housing and social services. Ideal for licensed LMHC professionals passionate about helping clients achieve independence and stability.

Responsibilities as the Mental Health Counselor (LMHC):

• Assessment & Service Planning: Conduct comprehensive mental health assessments and develop individualized service plans with long- and short-term therapeutic goals.
• Counseling & Support: Provide individual and group counseling, conduct home and office visits, and assist clients with treatment adherence, daily living skills, and resource navigation.
• Care Coordination: Collaborate with healthcare providers, social service agencies, and internal teams to ensure clients receive appropriate and timely services and follow-up care.
• Wellness Group Facilitation: Plan and facilitate weekly wellness/self-management groups to support client engagement and mental health education.
• Documentation & Compliance: Maintain accurate clinical documentation, including assessments, progress notes, and service plans, and participate in case conferences, trainings, and supervision.

Qualifications for the Mental Health Counselor (LMHC):

• Education: Master’s degree in Mental Health Counseling, Social Work, Psychology, or a related field required.
• Licensure: LMHC licensure required.
• Experience: 1-3 years of counseling experience in a mental health setting, preferably with HIV/AIDS or co-occurring mental health populations.
• Technical Skills: Strong knowledge of counseling practices, community mental health resources, and treatment compliance strategies.
• Skills & Attributes: Strong communication and organizational skills with a compassionate, culturally sensitive approach and the ability to manage multiple priorities in a community-based setting.
