JOB DESCRIPTION SUMMARY

Responsible for coordinating patients’ healthcare care needs within Bakersfield Community Healthcare’s service offerings, including but not limited to Home Health, Hospice, Adult Healthcare Center and Caregiving. Will develop connections with clients during the referral process and establish supportive relationships with clients upon discharge from agency services. Will work to ensure smooth transitions for clients across care settings to ensure ideal level of support is provided and maintained as appropriate. Developing and maintaining positive relationships with clients/patients and community healthcare providers is a primary requirement within this role.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1. Coordinate and collaborate with BAKERSFIELD COMMUNITY HEALTHCARE skilled nursing teams to identify patient healthcare needs.

2. Provide patients with the knowledge and understanding of BAKERSFIELD COMMUNITY HEALTHCARE services.

3. Educate patients regarding their current condition, medication, and other health care needs, and provide support and assistance whenever possible.

4. Maintain accurate patient records to ensure patient confidentiality and quality of care.

5. Work with BAKERSFIELD COMMUNITY HEALTHCARE Liaison team members on obtaining orders and records for new referrals.

6. Establish and maintain positive working relationships with all current and prior patients, along with current and potential referral sources.

7. Assist patients with scheduling appointments and obtaining new orders with healthcare providers as needed.

8. Manage communication between patients/families, healthcare providers, and case managers whenever a patient is discharged from BAKERSFIELD COMMUNITY HEALTHCARE services.

9. Provide community in-services regarding BAKERSFIELD COMMUNITY HEALTHCARE services.

10. Provide regular follow-up calls to check the needs and conditions of patients ensuring smooth coordination of care.

11. Report patient care needs to the appropriate personnel whenever additional and department specific follow-up is needed.

12. Consult and collaborate with other health care providers to establish appropriate treatment plans.

13. Ensure services comply with state and federal regulatory requirements.

14. Provide regular reports to management regarding patient status and/or concerns.

15. Provide leadership in strategic planning including identifying opportunities for additional or improved services to satisfy unmet customer needs.

POSITION QUALIFICATIONS

1. Graduate of an accredited practical nurse or vocational nursing program preferred, or prior experience or interest in a community-based healthcare setting, or prior experience or an affinity for health care marketing (Nurse or non-nurse acceptable).

2. Knowledge of corporate business management.

3. Demonstrates good communications skills, negotiation skills, and public relations skills.

4. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.

5. Ability to market aggressively and deal tactfully with customers and the community.

Job Type: Full-time

Pay: $20.00 - $30.00 per hour
