Nutrition Care Coordinator

The Nutritional Care Coordinator is responsible for accurate and efficient service of meals to the Health Center and Personal Care residents. This position includes assisting the Dietitian with clinical assessments of health care residents and supervision of the health care dining areas.
RESPONSIBLITIES
- Ensures direct reports follow correct punch in and punch out procedures.
- Demonstrated dependability with scheduled work hours.
- Ability to learn duties during on-the-job training.
- Responsible for orientation, training, supervising, evaluating (EDPs) and Performance Improvement Plans (PIPs) of all direct reports.
- Follow disciplinary action procedures for direct reports. Disciplinary action procedures to include verbal coaching and written warnings.
- Review EDPs and PIPs with direct reports after they have been approved by the Dietitian Manager/Senior Director of Culinary and or GM.
- Follow infection control practices and procedures for safety of residents, co-workers, and self.
- Maintain positive customer relations; handle complaints professionally.
- Participation in wait-staff training meetings.
- Enforce standards of service and proper uniforms of direct reports.
- Report any problems or concerns to direct supervisor.
- Assist in the planning of special events and pace changers.
- Responsible for providing excellent customer service to residents, visitors and staff.
- Attend monthly in-services and participate as a team-player in team discussions.
- Promote and support workplace diversity initiatives.
- Assist the Dietitian with assessments and diet history for all residents in the Health Center per company standard.
- Assist the Dietitian by maintaining accurate records of all interactions with residents. Consistently perform assessment communications in the medical record. Write clear and concise notes in the medical record after each visit.
- Attend Individual Care Plans per company standard as assigned.
- Supervise personnel in the healthcare neighborhoods by overseeing the quality and quantity of food prepared and served.
- Ensure accuracy of diet orders and monitor food intolerances, allergies, and preferences.
- Assume responsibility for food production system including entering food tallies on production sheets and HACCP record sheets and printing menus.
- Responsible for the Quality Management System by audit tracking and taking corrective action as needed.
- Floor stock and nourishment tracking.
- Responsible for cleanliness and sanitation of all healthcare pantry areas.
- Adhere to federal, state, and local guidelines and laws including but not limited to HIPAA, resident abuse, and Elder Justice Act.
- Exhibit behaviors set forth by the Code of Ethics and Business Conduct in the performance of their duties. Adhere to the Organization’s Policies and Procedures regarding the Compliance Program and is timely in reporting any possible concerns.
- All other duties as assigned by food service manager.
QUALIFICATION REQUIREMENTS:
- Associate’s degree in Food and Nutrition or a Certified Dietary Manager (or willing to obtain within the first year of employment).
- Willingness to perform repetitive tasks and possess cooperative, reliable attitude.
- Must have visual acuity to read production sheets, menus and recipes.
- Must possess maturity and willingness to assume responsibility as well as a co-operative attitude in person-to-person relationships.
- Demonstrate dependability and consistency with all policies and procedures in the Dining Services department.
- Must have the ability to interact well with residents, visitors and staff and be able to resolve any customer complaints or re-direct to the appropriate manager.
- Must possess exceptional customer service skills.
- Ability to communicate and interact with staff from all departments.
- Must practice a clean as you go philosophy.
- Must be able to enforce policy and procedures.
- Must be highly organized.
- Must possess self-direction in management of time, work & related activities.
- Must be able to read, write, understand & follow written & oral instructions.
PHYSICAL REQUIREMENTS:
Physical demands: While performing the duties of this job, the employee is required to stand, walk, and sit; use hands to finger, handle, or feel objects, tools, or controls; reach with hands and arms; talk and hear. Specific vision abilities required by the job include close vision, distance vision, and color vision. Occasionally the employee must lift and/or move up to 30 pounds when working in the dining room environment. Prolonged exposure to computer display terminals may lead to eyestrain for staff who works with computers.
Working Conditions: Nutritional Care Supervisor works in an office setting approximately 50% of the scheduled work day. The remaining 50% of the work day is on the floor meeting with nursing staff, residents and or family members in resident rooms, or in dining room environments.
