Comprehensive Care Management, a nonprofit 501(c)(3) organization affiliated with Network Solutions IPA (NSIPA), is seeking a highly motivated and experienced Lead Care Manager to oversee our care coordination programs, with a strong focus on Chronic Care Management (CCM) and Remote Patient Monitoring (RPM).
This leadership role is ideal for a licensed clinician who is passionate about improving patient outcomes through innovative care delivery models and scalable, technology-enabled interventions.
???? Key Responsibilities- Leadership & Supervision:
- Oversee a team of care coordinators, RNs, social workers, and other care management staff.
- Provide training, performance monitoring, and ongoing support for care team members.
- Chronic Care Management (CCM):
- Lead implementation and oversight of Medicare-compliant CCM services.
- Coordinate care plans for patients with multiple chronic conditions.
- Ensure accurate documentation, billing compliance, and quality reporting.
- Remote Patient Monitoring (RPM):
- Manage RPM workflows, device deployment, and follow-up protocols.
- Monitor patient data and alert clinical staff to potential issues.
- Work closely with tech vendors and providers to ensure patient adherence and safety.
- Program Development:
- Design and scale population health programs addressing SDOH, high-risk patients, and care transitions.
- Collaborate with partner organizations, including hospitals, payers, and physician groups.
- Compliance & Quality:
- Maintain compliance with CMS regulations, HIPAA, and clinical best practices.
- Track KPIs and report performance metrics related to patient outcomes, utilization, and satisfaction.
- 3+ years of experience in care management or population health.
- 1+ year of leadership/supervisory experience in a healthcare setting.
- Hands-on experience with CCM, RPM, and value-based care models.
- Proficiency in EHRs, care coordination platforms, and remote monitoring technologies.
- Excellent communication, leadership, and problem-solving skills.
- Bilingual a plus (Spanish, Russian, Haitian Creole, etc.).
