About Us

At Holly Hughes & Co., we approach care differently — and we like it that way. Our Care Managers bring a deep commitment to lead with empathy while supporting people and families navigating the complex intersections of health, mental health, substance use, and social needs.

We follow a Quadruple Aim framework and all our team members have guiding principles for the shared work we do. In this role, here are those guiding principles in action:

  • Quality: You use data-driven, outcome driven approach to care

  • Reach: You help more people access the right care from the right person at the right time

This role is part of our Enhanced Care Management (ECM) team — a bold new service line designed for our patients who face multiple barriers to wellness.
