Job Title: Hospice Medical Social Worker
Job Type: Part Time / 24 hr.
Location: Framingham, MA and surrounding areas
Reports To: Hospice Clinical Manager
Salary Range: Starting at $40/hr.
Job Summary:
The Hospice Medical Social Worker provides emotional support, counseling, and resource assistance to terminally ill patients and their families. Working as part of an interdisciplinary hospice care team, the social worker helps patients navigate the psychological, social, and financial challenges associated with end-of-life care.
Key Responsibilities:
- Conduct initial psychosocial assessments of patients and families to determine emotional, social, and environmental needs.
- Develop and implement individualized care plans in collaboration with the hospice team.
- Provide counseling to patients and family members to help cope with terminal illness, grief, and bereavement.
- Educate families about the dying process, hospice philosophy, and available community resources.
- Assist patients and families with advance directives, funeral planning, and other end-of-life decisions.
- Connect families with community services such as financial aid, housing, and mental health support.
- Collaborate with nurses, physicians, chaplains, and other team members to ensure coordinated and compassionate care.
- Maintain accurate and timely documentation of patient interactions, care plans, and progress notes.
- Participate in interdisciplinary team meetings and contribute to care planning discussions.
- Provide crisis intervention as needed.
- Offer bereavement support to families following the patient's death.
Qualifications:
- Education: Master's degree in social work (MSW) from an accredited school preferred
- Licensure: Licensed Clinical Social Worker (LCSW) or equivalent licensure as required by state law.
- Experience: Prior experience in hospice, palliative care, medical social work, or a related field preferred.
- Skills:

- Ability to work independently and as part of a team
- Knowledge of community resources and entitlement programs
- Documentation and case management proficiency
Work Environment:
- Typically works in patients' homes, hospice facilities, hospitals, or nursing homes.
- Will require travel between patient locations.
- Must be comfortable working in emotionally challenging end-of-life situations.
Must be a Licensed Social Worker in the service area.
