Job Summary

The LVN Lead Care Manager will play a vital role in coordinating patient care across healthcare settings. This position combines clinical nursing expertise with case management principles to ensure patients receive appropriate, timely, and cost-effective care. The LVN Case Manager collaborates with physicians, social workers, and other healthcare professionals to develop and implement individualized care plans.

Key Responsibilities

• Conduct comprehensive assessments of patients' medical, psychosocial, and functional needs
• Develop and implement individualized care plans in collaboration with interdisciplinary teams
• Monitor patient progress and adjust care plans as needed
• Facilitate transitions of care, including discharge planning and referrals to community resources
• Perform utilization reviews to ensure appropriate levels of care
• Educate patients and families on treatment plans, medications, and follow-up care
• Maintain accurate and timely documentation in electronic health records
• Advocate for patients to ensure access to necessary services and support
• Participate in quality improvement initiatives and case review meetings

Qualifications

• Active LVN license in California
• Minimum of 2 years of clinical nursing experience, with at least 1 year in case management preferred
• Strong understanding of healthcare systems, insurance plans, and community resources
• Excellent communication, organizational, and critical thinking skills
• Proficiency in electronic medical records (EMR) and case management software

Benefits

• Base Salary: $35.00 an hour
• Benefits: health insurance, dental and vision after 60 days of probation period, employee pays 50% premium.
• Working Hours: FULL TIME 9AM TO 1PM (lunch 1 to 2) 2PM-6PM
• Vacation: 40 hours per year and 40-hour sick time; accumulated at 1 hour per 30 hour worked. Ten paid holidays. (Thanksgiving day and the Day after, Christmas Eve and Christmas day, New Year’s day, Martin Luther King’s birthday, Juneteenth, 4 of July, Labor Day, and Memorial Day)
• Mileage reimbursement, according to IRS rule of the year
