The Villa at Texarkana is looking to add a Licensed Social Worker to our team!

This role is responsible for weekly MDS assessments (Cognitive Assessment, Mood Assessment, and Smoke Assessment); scheduling Care Plan Meetings; coordinating in-house or community appointments for psych, dental, vision, etc as needs arise; arranges Home Health or Hospice for resident discharges as needed; serves as a resource for all resident needs.

Our goal is to provide exceptional care to our residents. The Licensed Social Worker must be exceptionally organized and efficient, with excellent communication and problem solving skills. Must have computer knowledge in Word, Excel, etc.

Long-term or medical field related experience desired.

Monday-Friday, 8:00am-4:30pm
