Position Summary

We are seeking a licensed LPN/LVN to coordinate care for patients enrolled in our Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs in the Tampa Bay area. This hybrid role combines direct patient care support with local clinic outreach. You will work as part of a collaborative remote care team while serving as ColigoMed’s local representative with assigned clinics. The role requires a balance of clinical expertise, patient engagement skills, and professional representation of ColigoMed to healthcare providers.

The hybrid role will involve working from home for around 2-3 days per week with the remaining time being spent on site at clinics in the Tampa Bay locality.

• Must reside within a commutable distance to Tampa Bay, Florida.

Key Responsibilities

Patient Care & Clinical Support

• Manage ongoing relationships and communication with assigned patients.
• Follow up on patient needs, coordinate care plans, and monitor medication compliance.
• Educate patients on using the ColigoMed app and RPM devices, assisting with setup and first readings.
• Document interactions, notes, and progress accurately in the ColigoMed portal.
• Escalate clinical concerns promptly to the assigned healthcare provider.
• Collaborate with RPM/CCM providers to share patient updates and follow-up needs.

Clinic Outreach & Representation

• Serve as the local point of contact for assigned clinics.
• Represent ColigoMed professionally when engaging with new clinics in Tampa Bay area.
• Support efforts to onboard new provider partners in a credible and respectful manner.
• Build and maintain strong relationships with healthcare providers and clinic staff.

Requirements

Education & Experience

• Licensed Practical Nurse (LPN/LVN) with an active compact multistate license.
• 1-2 years of clinical nursing experience, ideally in chronic care management or patient monitoring.
• Experience representing a healthcare provider, service, or program to external clinics or partners.

Other Requirements

• Must reside within a commutable distance to Tampa Bay, Florida.
• Background check required.
• Dedicated home workspace with high-speed internet for remote work.
• Access to a reliable vehicle and willingness to travel locally.
• Typical schedule: Monday to Friday, 8:00 a.m. – 5:00 p.m. (1-hour lunch), with flexibility based on clinic operations.
• Contractor is responsible for own taxes and benefits under the 1099 arrangement.

Compensation & Contract Details

• Hourly rate: $21.00 – $25.00/hour (based on qualifications and experience).
• Mileage reimbursement for approved travel between clinics.
• Contract type: 1099 independent contractor.
• Potential for conversion to full-time employment after 6–12 months based on performance and business needs.
• If converted to full-time, the role will be eligible for company employee benefits (Medical, Dental, Vision insurance, Paid Time Off, Stock Options, and career advancement opportunities) per ColigoMed policies in effect at that time.
