Requirements:
Current Social Work license in the state of Texas
Bachelor's Degree in Social Work. One year of supervised social work experience working directly with individuals in a long-term or sub-acute setting
As a member of the rehabilitation team, the Licensed Social Worker provides services to address the emotional needs of residents and their families. This employee also assists with the resident admission and referral process.
- As part of the admission process, the licensed social worker will meet with the resident and family to review policies on advance directives, residents' rights, and discrimination.
- The licensed social worker will conduct and document a social services evaluation, including the identification of the resident's problems and needs.
- The licensed social worker will provide/arrange for social work services as indicated by the resident's/family's needs
- The licensed social worker will assist residents, as needed, with financial planning
- The licensed social worker keeps the resident's families informed of problems, personal needs, room transfers, and changes of level assignments
- The licensed social worker maintains a current list of community resources to facilitate referrals
- The licensed social worker demonstrates assessment skills sufficient to evaluate residents' behavior, to collect data to evaluate psycho-social needs, risk factors for psycho-social deterioration, and residents' responses to interventions. Able to interpret and explain resident behavior to staff in a way that fosters understanding, facilitates treatment, and respects resident rights and minimizes the use of chemical and physical restraints
- Able to implement interventions that achieve treatment goals, address residents' needs, link social support, physical care, and physical environment to enhance quality of life
- Participates in quality improvement activities, which reduce cost and increase efficiency through improved systems and processes.
