About Job

Woodcrest Rehabilitation & Residential a facility located in Flushing, NY is seeking Full-time Social Worker (BSW or MSW).
  • Long Term Care/ Subacute experience preferred.
  • Attend and participate in interdisciplinary care plan meetings.
  • Complete MDS while developing and maintaining individual care plans.
  • Interview and access residents’ needs during the admission interview process.
  • Work with the team to coordinate discharges.
  • Work with residents to maintain psychosocial well-being.
