Job Title: Supervisor, Social Work

Location: Hybrid (Alexander Park and occasionally St. Mary's)
Hours Per Week: 40
Schedule: Monday-Friday, 8am-4:30pm
Summary
Provides staff supervision related to assigned areas of responsibility. Supervisor, Social Work, works closely with Department /Practice Managers to ensure positive working relationship between social work staff and RRH Hospital/Medical Group providers and staff, as well as community partners. Adheres to policies, procedures, and established standards of professional conduct. Upholds patient rights in all settings.

Physical Requirements
S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.

Summary

Responsibilities
- Provides direction in performance standards, expectations, and productivity for each staff member.
- Develops quality indicator to measure performance and outcomes.
- Coordinates and monitors all in-reach and outreach services provided by the social work team.
- Supervises assigned social work staff.
- Selects appropriate candidates for positions, working closely with the appropriate leader.
- Provides for orientation of new staff. Supervises and mentors staff, reviews staff performance with appropriate leader, and completes required performance appraisals.
- Delivers ongoing education, training, and motivation to empower all team members for when social work staff and/or leader are unavailable.
- Monitors internal and external referrals to identify opportunities for increased utilization of RRH resources as well as address any issues that might arise during process. Conducts, monitors and responds to Safe Connect incidents.
- Master’s Degree in Social Work.
- Valid NYS Driver’s License.
- LCSW
- Strong analytical and communication skills
- Proficiency in Microsoft Office products and ability to work various software applications.
- Experience in the Medicaid eligibility determination and enrollment process.
- Five years’ experience in healthcare, social work or related position, with supervisory experience preferred.
