Job Title: Mental Health Services Coordinator
Location: Bronx, NY 10461
Pay Rate: $33 per hour
Schedule: Monday to Friday, 9:00 AM to 5:00 PM
Employment Type: Full-time - Temp to Perm

About the Role: Mental Health Services Coordinator to join a hospital-based care transition team at a Bronx Psychiatric Center. This team helps adults with serious mental illness prepare for discharge and supports them as they transition back into the community.

Responsibilities:

• Provide short-term counseling and support on the hospital unit and in the community
• Run support groups and teach daily living and coping skills
• Help individuals apply for housing and attend interviews
• Complete assessments and build care plans
• Accompany individuals to appointments and connect them with services
• Communicate with hospital staff, housing providers, and support networks
• Be part of an on-call rotation for after-hours support

Requirements:

• Master's in Social Work, Mental Health Counseling, Psychology, or Nursing
• 2 years of experience in mental health or criminal justice settings
• Experience with group work, assessments, and discharge planning
• Comfortable working both onsite and in the community

Preferred:

• Bilingual in English and Spanish # 25-13967
