Organization Overview:

Compass House provides safe shelter and services for runaway, homeless, and at-risk youth in Western New York. Our Rapid Re-Housing (RRH) program helps young people ages 18–24 transition from homelessness to stable, independent living through housing placement, financial assistance, and ongoing supportive case management.

Position Summary:

The Rapid Re-Housing Case Manager supports young adults in securing and maintaining permanent housing. Using a trauma-informed, client-centered approach, the Case Manager helps youth develop essential life skills, connect with employment and education opportunities, and access supportive services that promote long-term housing stability.

Key Responsibilities:
  • Conduct client intakes, assessments, and develop individualized housing and service plans.
  • Assist clients with locating safe and affordable housing; support lease negotiations and move-in coordination.
  • Provide consistent case management services, including home visits, goal setting, budgeting, and crisis intervention.
  • Collaborate with landlords, property managers, and community partners to support successful tenancy.
  • Maintain accurate and timely documentation in HMIS and internal databases.
  • Coordinate access to resources including mental health services, employment programs, benefits, and transportation.
  • Work in partnership with the Rapid Re-Housing Director and team to meet program goals and comply with funding requirements.
  • Attend staff meetings, trainings, and community provider meetings as needed.

Qualifications:
  • Bachelor’s degree in Social Work, Human Services, Psychology, or a related field required.
  • Strong understanding of trauma-informed care, youth development, and housing-first principles.
  • Excellent communication, time management, and organizational skills.
  • Must pass NYS background check.
  • Proficiency in Microsoft Office and ability to learn database systems (e.g., HMIS).
  • Valid NYS driver’s license and reliable transportation required for regular home visits and community-based work.
