About Job

Our Service Coordination agency based in Spring Valley, NY is experiencing major growth, and we are seeking Nursing Home Transition and Diversion (NHTF) Waiver Services Coordinators to help Medicaid-eligible seniors and people with physical disabilities receive comprehensive quality services in a community-based setting.


Service area currently includes NYC and Lower Hudson Valley counties. The role is full-time, Monday-Friday. The position is fully remote; however qualified candidates must have access to reliable transportation as this position requires the Service Coordinators to visit participants in the home and at various locations once every three months. A valid driver’s license is preferred.

Job Type: Full-Time - Remote with some field travel

Salary Range: $55,000-$65,000 annually

  • Eligible candidates must be located in the NYC Metropolitan Area.

You will:

  • Help individuals access Medicaid and other vital services
  • Create, coordinate, and monitor person-centered service plans
  • Assist individuals transitioning from nursing homes into the community
  • Support those already living in the community to maintain independence
  • Conduct field visits to evaluate and ensure participant safety

You will have:

  • Strong communication and people skills
  • Must be self-directed yet collaborative
  • Reliable, supportive, and driven to make a difference
  • Organized and deadline-oriented to keep participants on track with eligibility requirements
  • Must be computer literate
  • English proficiency is a must, and additional languages are a plus
  • Ability to travel to participants homes at least once every three months; a valid driver’s license is preferred
  • Bachelor’s degree or certification in social work, psychology, health care management or related health science field is preferred
  • A minimum of 3 years of experience in providing case management or social work services for elderly or disabled individuals
  • Experience providing compassionate, qualified support in helping individuals navigate care options, access services and live safely and productively within their community
