Position Overview
The Care Coordinator serves as a key liaison between families, Respite Care Providers, and internal teams. This role manages a caseload, coordinates scheduling, ensures documentation compliance, and supports service utilization. Success in this position requires strong organizational skills, attention to detail, critical thinking, and a commitment to high-quality, responsive care.
Key Responsibilities
Serve as the main point of contact for families and providers, handling inquiries, schedule changes, and support needs.
Maintain documentation compliance (IPPs, Parent Packets, Conditions of Employment, EVV signatures); update internal systems regularly.
Review and reconcile paper and electronic timecards; investigate and resolve discrepancies.
Fill open shifts, track provider availability, and manage staffing coordination; escalate unresolved gaps to leadership.
Monitor service utilization (overages, under-utilization, inactive clients) and maintain accurate documentation in WellSky.
Collaborate with the EVV Clerk to ensure compliance with visit verification records.
Document and escalate Special Incident Reports (SIRs); coordinate with CPS/APS as needed.
Manage client terminations and replacements; communicate with Regional Center CSCs.
Maintain scheduling systems, shift tracking, and assist families with tech issues or system navigation.
Process service authorization changes per established guidelines.
Keep detailed notes and case documentation to ensure regulatory compliance and continuity of care.
Escalate complex concerns to the Senior Care Coordinator and assist with departmental projects or administrative duties as assigned.
Required Qualifications
Associate's degree or equivalent work experience in Human Services or related field
Bilingual in English and Spanish (required)
Familiarity with developmental disabilities (preferred)
Strong confidentiality and accuracy; HIPAA compliance required
Proficient with Microsoft Office, EHR platforms, and cloud-based systems
Excellent verbal/written communication, organization, and time management skills
Reliable transportation: valid driver's license and auto insurance required
Work Schedule & Physical/Environmental Requirements
Full-time: MondayFriday, 8:00 AM 5:00 PM with a 1-hour lunch
Must be flexible to work beyond scheduled hours as needed
Requires sitting for extended periods, frequent computer use, and occasional lifting (up to 25lbs.)
Physical activity includes walking, bending, twisting, and carrying supplies
Work occurs in both office and home visit environments with varied indoor/outdoor conditions
