Job Summary


The Housing Case Manager provides case management services to women residing in the YWCA NENY housing program. The program serves survivors of domestic violence, women with varying levels of mental health diagnoses, women recovering from addiction, those recently released from incarceration, individuals living in extreme poverty, chronically homeless individuals and other marginalized populations.

Reports to: Director of Housing

FLSA Category: Non-exempt


Essential Duties:

  • Provide clients with housing information, emotional support, referrals, and guidance throughout the housing process.
  • In collaboration with the Housing Director, conduct housing intakes, manage the housing waitlist, and assist with intakes for the Schenectady Coordinated Entry system.
  • Maintain up-to-date knowledge of local social services, YWCA rental procedures, and tenants' rights and responsibilities.
  • Track and document client data, including daily resident tracking, case management notes, and client files; compile statistical data and generate reports related to homeless services.
  • Work closely with the Director of Housing and the YWCA maintenance team to establish and maintain safe and supportive living environments for program participants.
  • Facilitate monthly group activities to promote community engagement and client well-being.
  • Conduct room checks alongside another case manager or the Director; assist with room turnovers as needed.
  • Provide transportation for clients to appointments using the agency van.
  • Attend staff meetings, case management meetings, and required trainings.
  • Occasionally provide front desk coverage.
  • Perform other duties as assigned to support the program and agency operations.


Qualifications:

  • High School Diploma/GED; Associate’s degree in human services or related field preferred.
  • Clean, NYS Driver’s license and ability to use own vehicle for work-related travel
  • Proficiency with Microsoft Office, Internet use, and data base use
  • Proficient problem-solving and communication skills


Physical Requirements

  • Prolonged periods of sitting at a desk.
  • Ability to use computer and necessary agency software.
  • Able to lift up to 25 pounds without team assistance.
  • Occasional bending, standing, reaching, stair climbing.

Additional Requirements

  • Complete all role required checks.
  • Clean NYS Driver’s license and Traffic Abstract.
  • Ability to use own vehicle for work-related travel.
