Overview
The Care Coordinator plays a vital role in ensuring the well-being and quality of life for individuals in various care settings, including assisted living, nursing homes, and memory care facilities. This position involves working closely with clients, families, and healthcare professionals to develop and implement personalized care plans. The ideal candidate will have a strong background in senior care, direct support, and experience working with individuals with developmental disabilities or dementia.
Responsibilities
- Collaborate with healthcare providers to create and manage individualized care plans for clients.
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, meal preparation, and medication administration.
- Provide compassionate support to individuals with Alzheimer's, dementia, or other cognitive impairments.
- Conduct regular assessments of client needs and adjust care plans accordingly.
- Maintain accurate documentation using EMR systems while adhering to HIPAA regulations.
- Facilitate communication between clients, families, and interdisciplinary teams to ensure comprehensive care.
- Manage conflicts effectively and provide behavior management strategies as needed.
- Support clients in achieving their personal goals through case management and direct support services.
- Assist with vital signs monitoring and basic first aid when necessary.
- Ensure a clean and safe environment for clients by performing light cleaning duties.
Qualifications
- Previous experience in assisted living, nursing homes, or home care settings is preferred.
- Knowledge of medical terminology, patient care practices, and caregiving techniques.
- Familiarity with working with individuals with disabilities, including autism and developmental delays.
- Strong interpersonal skills for effective communication with clients and their families.
- Ability to perform heavy lifting and assist clients using equipment such as Hoyer lifts when required.
- Experience in conflict management and behavior management is a plus.
- Valid driver's license may be required for transportation purposes.

Pay: From $23.00 per hour
