RESPONSIBILITIES
Under the supervision of the Program Coordinator, the care coordinator will assist individuals, children, adolescents, and adults, who have presented to Rapid Access and did not meet criteria for active on-going services. The Rapid Access Care Coordinator will triage and then provide linkage, appropriate assessments and collaboration with community resources to assist the individual in receiving the appropriate behavioral services. Provides specialized services to meet a variety of individual needs such as, but not limited to, housing, education, risk assessment, and prevention of infectious disease. Sessions may be individual, group, and family settings on-site.
REQUIREMENTS
Minimum Requirements
M1: Bachelor’s degree in psychology, social work, criminal justice, vocational rehabilitation, counseling, sociology, or other approved human services degree,
OR
Certified psychiatric rehabilitation practitioner in Virginia
M2: Valid driver’s license and safe driving record
Fingerprints, State and FBI criminal record reports, drug test, and central registry (CANIS) report will be required upon request. Our Agency maintains a drug-free workplace.
ANNUAL SALARY RANGE
$43,722 - $76,514
