Your Role: Care Manager
Join our team as a Care Manager, where you'll assess and support clients' psychosocial, physical, mental, and environmental needs. You will develop client-centered care plans, conduct home visits, and facilitate access to community services. This role involves collaborating with clients, caregivers, and healthcare providers, ensuring timely and appropriate care. You will document case management activities, provide crisis intervention, advocate for clients, and participate in ongoing quality improvement. A strong understanding of Medicare and Medi-Cal is highly preferred.
What You’ll Do
Key Responsibilities:
- Conduct comprehensive assessments and develop individualized care plans
- Coordinate and monitor community services, ensuring quality and timeliness
- Maintain client relationships through regular visits, advocacy, and support
- Document all case management activities and ensure continuity of care
- Collaborate with healthcare providers and community partners
- Participate in team meetings, ongoing training, and quality improvement initiatives

What We’re Looking For
- Educational Background: Bachelor's degree in social work or related field.
- Experience: At least one year of experience working with disabled adults and/or older adults. Experience with mental and behavioral health diagnoses and substance abuse disorders is highly desired.
- Skills: Exceptional communication and presentation skills. Strong case management skills and experience in the community health care delivery system. Detail-oriented with good problem-solving skills and the ability to prioritize multiple tasks.
- Attributes: Compassion, patience, and a genuine desire to make a difference in the lives of others.
Physical Requirements
- Communication: Ability to talk and hear during face-to-face communications.
- Mobility: Ability to reach overhead and below the knees, including bending, twisting, pulling, and stooping.
- Coordination: Multi-limb and eye-hand coordination.
- Endurance: Ability to sit at a desk for prolonged periods while working on computers. Ability to move, lift, carry, push, pull, and place objects weighing more than or equal to 40 pounds without assistance.

Pay: $68,000.00 - $72,000.00 per year
