Responsibilities:

  • Assess the psychosocial functioning of Palliative and Home Health care patients and families in their home environment and develop an individualized plan of care
  • Provide appropriate psychosocial interventions in accordance with the physician's orders and the policies and procedures of the Agency
  • Educate patient/family on, and assist in, preparation of advance directives
  • Provides information and referral to patients and families/caregivers regarding necessary community resources
  • Maintains clinical records on all patients referred to social work prepares clinical and progress notes
  • Function as an integral member of the interdisciplinary team
Requirements:

Qualifications:

  • Master of Social Work Degree
  • New Grads welcome, we will train the candidate; clinical supervision is available
  • Bilingual in Spanish and English
  • People-oriented self-starter; notable written and verbal communication skills, and customer service skills
  • California licensed and insured driver with reliable transportation
