Job Overview
We are seeking a dedicated and experienced Social Work to lead our team in providing exceptional care and support to our clients. The ideal candidate will possess a strong background in social work, behavioral health, and mental health counseling, with a focus on patient assessment and case management.

Responsibilities

• Conduct patient assessments to determine needs and develop appropriate treatment plans.
• Oversee the implementation of individual and group counseling sessions, ensuring adherence to best practices in behavioral therapy.
• Maintain accurate medical documentation and records in compliance with HIPAA standards.
• Facilitate intake processes for new clients, ensuring thorough evaluations are conducted.
• Provide clinical counseling and support for patients dealing with mental health issues, aging concerns, and other behavioral health challenges.
• Lead group therapy sessions that promote healing and recovery among participants.
• Collaborate with healthcare professionals to ensure comprehensive patient care across various settings.
• Stay informed about the latest developments in social work practices, medical terminology, and geriatric care.

Skills

• Proficiency in behavioral therapy techniques and applied behavior analysis.
• Familiarity with medical documentation processes and terminology.
• Experience in managing medical records and maintaining confidentiality per HIPAA regulations.
• Knowledge of intake procedures and diagnostic evaluation methods.
• Ability to provide effective case management for diverse populations.
• Experience in individual/group counseling within a behavioral health context.
• Understanding of home & community care experience is preferred..
• Competence in psychodynamic therapy approaches and aging issues counseling.

Job Type: Full-time

Pay: $30.00 per hour
