The Discharge Planner is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources, balanced with the patient’s resources and right to self-determination.
QUALIFICATIONS
This position will provide clerical support to the Case Managers and Social Workers. B.S. Sociology / Psychology is required with a minimum of 1 year working in a behavioral health environment.
Shall have knowledge and experience of confidentiality and compliance with HIPAA regulations.
Skills/Knowledge/Ability: Medical or hospital experience preferred, basic Microsoft computer skills, ability to maintain a work pace appropriate to the workload and must demonstrate customer service skills appropriate to the job.
BLS (American Heart Association) for healthcare providers upon hire and CPI certification within 30 days of hire.
MAJOR TASKS, DUTIES AND RESPONSIBILITIES
· Collaborates with the physicians, therapists, treatment team, and patient in order to develop effective continuing care plans for patients.
Makes telephone calls, faxing, copying and coordinating discharge plans.
· Provides patients with reasonable aftercare options and discusses options with the patient individually and/or with family, as requested by the patient.
· Collects releases of information to accurately research and refer to services.
· Maintains timely and accurate records.
· Initiates contact with referral sources, community mental health providers, physicians, nursing homes, residential treatment facilities, halfway houses, and other placement options to facilitate continuity of care.
· Coordinates admissions to secondary hospitals, treatment centers, nursing homes, halfway houses, and other placement as needed.
· Schedules follow up appointments with psychiatrist and therapist for continued treatment as an outpatient whenever possible
· Attends and contributes to treatment team meetings in a punctual and professional manner.
· Maintains positive relationships with community resources/referrals, and represents the facility in a professional manner.
· Actively researches and updates provider, treatment facility, and halfway house lists.
· Maintains an up-to-date referral, contact, and resource database.
