The Intake Coordinator serves as the first point of contact for individuals inquiring about hospice services. This role is responsible for gathering referral information, coordinating with the admissions team and ensuring a smooth and efficient admission process. The Intake Coordinator plays a vital role in communicating with patients, families, healthcare providers and internal staff to facilitate timely access to compassionate end-of-life care.

Minimum Qualifications:
- High School diploma or GED required; Associate’s or Bachelor’s degree in healthcarerelated field preferred.
- 13 years of experience in healthcare intake, admissions or customer service; hospice or home health experience preferred.
- Familiarity with medical terminology and EMR systems.
- Must have excellent verbal and written communication skills and be able to communicate effectively with individuals under stress.
- Excellent organizational and computer skills required.
- Strong attention to detail and data accuracy.
- Ability to multitask and work in a fastpaced environment.
- Compassionate, patientfocused approach.
- Receive and process incoming hospice referrals from hospitals, physicians, facilities and families.
- Demonstrates ability to explain Montgomery Hospice services and programs, hospice philosophy and eligibility criteria to all who call requesting information.
- Obtain and verify all necessary demographic, clinical and insurance information to determine hospice eligibility.
- Enters and updates patient information accurately in the Electronic Medical Record (EMR) system.
- Records and tracks referrals through admission or termination of the referral.
- Communicate with referral sources to obtain medical records and clinical documentation.
- Provides information about payment provisions for Medicare, Medicaid and commercial insurance during admission process.
- Works cooperatively with Insurance Coordinator when verification of insurance coverage is needed.
- Collaborate with the admissions team to schedule admissions.
- Serve as a liaison between referral sources and the hospice admissions and clinical teams.
- Develops an effective working relationship with representatives from other agencies who provide services for assigned patients.
- Maintain accurate logs, reports and documentation in accordance with organizational policies and regulatory requirements.
- Ensure compliance with all HIPAA, Medicare, Medicaid and other regulatory guidelines.
