About the Role:

The Care Coordinator plays a crucial role in ensuring that patients receive comprehensive and coordinated care throughout their healthcare journey. This position involves collaborating with healthcare providers, patients, and their families to develop and implement individualized care plans that address medical, emotional, and social needs. The Care Coordinator will monitor patient progress, facilitate communication among all parties, and advocate for patients to ensure they receive the necessary resources and support. Minimum Qualifications:
