About Job

OCHS operates several Community Health Centers throughout Osceola County that are accredited by The Joint Commission for Ambulatory Care and Primary Care Medical Home.

This is highly responsible and independent position reporting directly to the CMO. The Licensed Mental Health Counselor (LMHC) works as part of a multidisciplinary team to case manage a panel of patients who require mental health and substance abuse services. Services include assessment, diagnosis and treatment planning of behavioral health for children, adolescents, adults, and families in order to improve psychosocial functioning. Works closely with the Physician, Psychologist and Registered Nurse on behavioral health issues.

Addresses referrals from providers. Provides therapeutic services to patients and their families. Observes client behavior, assesses needs and develops treatment plans and strategies. Diagnoses psychological, behavioral and emotional disorders. Completes required documentation and paperwork in a timely manner. Maintains accurate and timely records, including clinical progress notes, according to organizational policy during the course of patient’s treatment. Consults with doctors, psychologist and medical professionals when appropriate.

Provides positive customer relations in dealing with patients, families, physicians, member of the healthcare team, referral sources, services agencies and others.

Performs all other duties as assigned.

Master's degree in mental health counseling and Florida license to practice

Minimum of two years work experience as a LMHC

Must have the ability to work effectively independently and in a team environment

Excellent interpersonal skills

Bi-Lingual English/ Spanish, preferred.
