Summary:
The JFS Aging Care Manager supports adults aged 60 and older— on-site at Ellison Park Apartments’ Naturally Occurring Retirement Community (NORC) by developing personalized care plans that promote safety, wellness, independence, and future planning. Through home visits, care coordination, transportation, home modifications, wellness initiatives, and social engagement, the NORC model helps older adults age in place while remaining connected to their community. The Care Manager also works closely with a diverse population, including Holocaust survivors and their families to ensure access to culturally sensitive services, benefits, and programs. This role is part of a collaborative team committed to promoting dignity, autonomy, and enhanced quality of life for older adults.
Essential Duties and Responsibilities:
- Builds strong positive relationships with residents utilizing compassionate and responsive based skills.
- Maintains regular check-ins and on-going monitoring with residents following up on interests, wellness, social & cultural needs, concerns and advocacy.
- Provides short term case management regarding crisis support, referrals to community resources (e.g. transportation, health services, veterans services) assistance with benefits/entitlements.
- Collaborates with residents and their identified significant parties to support the resident’s highest quality of life.
- Conducts outreach within the Ellison Park Apartment community and residents.
- In conjunction with program director or as assigned conducts initial consultation/ assessment interviews.
- As assigned conducts and completes individualized care plans.
- Supports various social groups and activities and the Resident Planning Committee.
- ·Maintains timely, accurate and confidential documentation in accordance with agency and funder requirements.
- Actively participate in internal and external meetings, team case reviews, and professional development, including training on Person-Centered, Trauma-Informed (PCTI) best practices.
- Other duties as assigned by the supervisor.

Education and Qualifications:
- Bachelor’s degree in social work or a related field from an accredited college or university required.
- Fluency or proficiency in Russian as a second language is a plus.
- Minimum of 2 years of full-time experience in care management, program coordination, and client assessment. Must demonstrate the ability to evaluate individual and family situations, respond to crises, and assess safety concerns, including potential elder abuse or neglect.
- Strong knowledge of local community resources, including Monroe County Social Services, Social Security, and Mental Health Systems. Familiarity with services and interventions for older adults, particularly regarding financial and insurance-related challenges.
- Understanding of intergenerational trauma and the Holocaust is preferred.
- Effective interpersonal and communication skills, with the ability to conduct home visits comfortably and professionally.
- Highly organized, self-motivated, and capable of working both independently and as part of a team.
- Proficiency in electronic record-keeping systems, Microsoft Office 365, and Excel, with the ability to maintain timely and accurate documentation, forms, and statistical tracking.

Salary & Benefits:
This part-time (30 hours/week) offers an hourly rate between $22.00 - $25.40/hour.
