This position is 40 hours per week, working Monday through Friday from 800am-430pm, hours are flexible and hybrid schedule.

Responsibilities

  • Interacts with caregivers by phone/video/in person at Kennedy Krieger to assess patient and family needs and facilitate communication to/from multi-disciplinary team
  • Reads medical records and interacts with oncology team to gather information and discuss patient and family needs
  • Identifies appropriate patient/family referrals and resources and discusses with multi-disciplinary team
  • Provides referral information and other recommendations to caregiver
  • Documents in clinical and research records as needed.
  • Collaborates with the referral source, appropriate disciplines, and the family to plan the most effective evaluation or treatment program to meet proposed goals or outcomes.
  • Collaborates with payor source, referral sources, and Kennedy Krieger Institute Insurance Verification Specialists for financial approval of plans.
  • Provides ongoing monitoring of the patient’s progress toward identified goals by reviewing treatment plans, multi-disciplinary team recommendations, and/or progress notes. Utilizes a problem-solving approach to initiate and support changes needed to ensure quality, effective care.
  • Participates in multi-disciplinary team meetings .
  • Collects utilization data, as well as patient outcome measures, for ease of access, quality, and cost effectiveness.
  • Acts as an initiator and catalyst in the development of activities for innovative improvement of patient care outcomes.
  • Participates in internal and external marketing/public relations activities including disseminating information in informal and formal education programs.
  • Completes IRB trainings and retraining as required to be a research study team member for relevant protocols


Qualifications

Education Graduation from an accredited school of nursing. BSN or enrolled in a BSN program to be completed within 2 ½ years of hiring date.

Experience Minimum 3 years of related experience in a hospital or home care setting required.

License Active, unencumbered Registered Nurse license from Maryland or a compact state.

Certification Current CPR certification that included hands on skills.

Related case management, quality assurance, or utilization review certification preferred.
